Figured I'd write this since things were different than I was expecting based on the other reviewer. There are 3 base faculty orthopods at Millcreek, one just coming back a cpl years ago from a sports fellowship in UMass. But obviously lots of faculty I didn't meet since I was not able to travel to their main rotations (none of the core ortho rotations are actually at Millcreek). As an intern (I actually got to as a student), you work a lot with the (non-op) Sports Medicine attending and his fellows, doing clinic on the University D1-3 teams in Erie (Mercyhurst, Edinboro, Gannon), the NBA D-league teams, the OHL hockey team, and actually covering as the primary team doctor at games.
Didactics / Teaching
One full day a week of didactics, lit review, OITE questions, etc. Resident run for the most part, but while I was there the newer sports attending gave a series of lectures on throwing injuries and the PD gave a few sports specific lectures as well. Weekly sports radiology conference w sports med/radiology/ortho.
Operating Experience
As a student, was only able to see the cases at Millcreek. From PGY2 the residents begin rotating at the several UPMC hospitals in the (relatively close) area, a couple that are in Ohio (~2hrs), one in upstate NY, and one in WVU. From what I gather, the majority of these rotations have 2-3 residents (junior/senior); some told me they stuck with one attending for a period of time then switched, and others told me there were rotations where they'd round early and then divide up the cases that were on the board for the day. I had seen one of their residents in a hospital I was at in upstate New York working with a foot and ankle surgeon and they seemed to have great rapport and were always operating, but there is obviously much I haven't seen. The PGY5s that came back at the end of senior year to Millcreek (I think they all do at some pt in senior year) were all pretty competent and confident, with some obviously having better hands than others.
Clinic Experience
Best of all the DO programs I visited. The interns run their own orthopaedic clinic (it's a nice, shiny LECOM building in a way awfully dangerous neighborhood) for patients that don't have insurance, and they do/manage everything. One senior is always there to oversee but there is pretty huge autonomy from the get go. Afterwards, it is pretty attending dependent I heard.
Research Opportunities
Not so much research " opportunities", as much as research "REQUIREMENTS". I see this being a final arbiter for lots of applicants. This isn't just a residency program, it's a combined residency program with a MSc program, and there is a full research project due every year in order to get that Masters degree. Obviously a big time commitment, but the Millcreek ortho residents win awards at the academy regularly so I assume it pays off. To be honest, I figured the projects would be done just to fulfill requirements and move on, but lots of the ones I read were pretty legit, and a bunch of them got grants for it. But if research isn't your thing, it'll definitely have you think long and hard about ranking.
Residents
As I mentioned, all seemed like competent PGY5s when they came back. I think they're only graduating 3 this year (well I only know about 3 so I don't know), but of those, 1 accepted a job in his home town and the other 2 got into peds at Michigan, and trauma at York. Before them had one go to Hopkins for spine, one to UBuffalo for sports, and either Cleveland or Cincinnati for peds.
Lifestyle
One of the downsides in my opinion .. But might be someone else's positive aspect; a good majority of the residents have wives and kids. The fact that they have families and aren't all divorced is good for an orthopedic surgery program I think haha, and some of them came out to the bars on weekends with us, but not too many of them go out beyond the occasional golf or camping trip.
Location / Housing
Big Plus- there's housing provided at all the out rotations. The single dudes save lots of money just using the provided housing but I'd imagine it not being too perfect if you have a family with you. If you've never been to Erie, the lake is nice for about 3-4 months and there's plenty of golf and bar hopping to get into. But I would not be super happy about the winter time.

Travel, but housing is provided.

Overall Rotation Experience / Conclusion
The autonomy is appealing, the sports experience throughout intern year sounds fun, but I wish I was able to visit the rotation sites with them. The new attending from UMass was really easy to get along with and seemed to mentor the residents well, and there's rumors that he is in training to be the future PD. If you don't mind travel, love research, and want to go somewhere with a great track record of fellowship placement, then think about going up to Erie. As far as a LECOM preference that there is rumored to be, to be honest I have no clue if there is any, as you'll see there is one year where the PD didnt take any LECOM students, followed by another year where a majority of them are. Hard to tell.

two main attending at millcreek, both very knowledgeable, one a little nicer to work with than the other. both enthusiastic teachers

attempt some sort of teaching daily, but one half day devoted to teaching and fracture conference weekly, do it on video conference with residents on out rotations

very limited at millcreek, but you only spend one or two years at millcreek so cant comment on the out rotations which make up the majority of your training

lots of it, do office time with the two attendings
Research Opportunities
didnt see any, but im sure they are there as I think it is a mandatory thing for residents to graduate
Residents
most pretty nice and knowledgeable, though a couple who did not impress
Lifestyle
excellent for ortho, just not much to do at millcreek or in erie, at other sites im sure its worse
Location / Housing
erie is small town, about 100,000 total. there is a college so there are bars and a bit of a nightlife, lake is right there. buffalo and cleveland not far away
Limitations
lack of cases at millcreek, though they are talking of expanding to the other hospital which would be a huge boost to this program. too much traveling, not sure on research.
Overall Rotation Experience / Conclusion
had a good time but not my top choice. too much travel and not enough cases in erie. also not a fan of erie as a town. 5th years were impressive though so i think you will get good training. getting another site/attendings in erie would be huge
