Does anyone know if any programs simply decline to offer an interview but do not send a notification email?

Yes, some programs choose not to interview you and don't officially notify you of that fact. Not very polite, but that's the way the cookie crumbles.

You also have to keep in mind, however, that some programs have an "unofficial" waitlist...those ALSO don't offer you an interview and don't tell you no. That's why it's not over until you're told, "I'm sure you'll be a success somewhere else."

I think I remember from last year that several programs either never officially rejected the applicants, or they did it so late (like in January) that it was already quite obvious that the applicant wasn't going to get an interview.
