Have you (or do you know anyone) who has contracted for a position during residency that pays a stipend during residency? Any suggestions on how you approach opportunities?

For example, a guy in my program is only a third year and has already signed a contract to begin after he finishes fellowship. The position is already paying him a stipend as a resident.
