I'm fascinated as to how different centers manage the same injury/orthopaedic problem quite differently and so am curious for and invite you to submit your response. How would you/your institution manage a displaced subcapital neck of femur fracture in the following patients?. I'll give what would be a typical practice for my institution and would be interested to how yours would contrast:
(i) 30 yr old displaced subcapital neck of femur (nof) #: urgent ORIF with either dynamic hip screw or three cannulated AO screws.
(ii) Healthy 60 y.o " ": Total hip arthroplasty
(iii) Reasonable 80 y.o ": Uncemented modular HEMIarthroplasty - eg. Zwyemuller/Zimmer Alloclassic
(iv) Severly demented 95 y.o ": Cheap single-bloc (i.e non-modular) hemiarthrolplasty - typically Austin-Moore prosthesis.

We would pretty much go straight to arthroplasty and NOT attempt orif in completely displaced subcapital #'s in patients over 50.

Thanks.
