I am a rads resident trying to get some help on understanding your shoulder arthroscopy reports. When you guys talk about a clockface position such as on a labral tear, what is the standard terminology? I thought that anterior was from 1-5 o'clock position and posterior was 7-11 o'clock based on a lateral view. I have seen some surgical reports though that seem to indicate otherwise, but others that do not. Any further clarification would be helpful.

The glenoid clockface is with top (superior) of the glenoid being 12 and the most inferior 6. So the typically Bankart lesion in a right shoulder would be 3 - 6 ish and the SLAP 8 to 2. However, in a left shoulder some would say the Bankart zone is 9 - 6 since anterior and posterior are reversed. This is probably what is confusing for you.

thx for your reply. I am surprised that there is not standard terminology.

Personally I would prefer to use just quandrants such as anterior-superior, but it seems that most of the MSK radiologists prefer that clockface terminology because that is what you guys use and it is more precise.
