I have heard that they only send offers to those with AOA, are you AOA?

Nope. Top 10% of my class, but not AOA (my school has weird selection criteria). And I’m a re-applicant.

Yes.

I don't have the forms submitted yet, my impression was that wasn't necessary for the application to be complete. Does anyone know otherwise?

I missed the boat on the supplemental forms for Vandy, clue me in please.

We prefer to receive letters of recommendation from orthopaedic physicians with whom you have worked. In addition to your letters of recommendation, please ask each of your recommending physicians to complete one of the performance evaluation forms and submit it to our program. Should a physician opt not to complete the requested from, we would ask that his/her letter include the criteria addressed on our form. The first three letters received will influence our decision of whether or not to extend an invitation to interview.
