MD student at Low tier med school

I've been wanting to be an orthopod since I was in HS. I recently went through the MD admissions cycle and although I interviewed at some top tier med schools, I struck out and will be matriculating at a low tier private med school. With step 1 going pass fail, how much will my low school rank hurt me? What can I do to best prepare a competitive application?
