I did not match into Orthopedics last near and now I am about to start my gen surgery prelim year. As I begin the process for reapplying to orthopedics, I am looking for any advice as too which programs I should apply to?
Do you know which programs are more likely to accept someone on their second try?
Should I apply in the region I grew up and attended medical school or in the region where I am doing residency?
Is it frowned upon to have a back up plan?

My advice: Apply everywhere. Matching the second time through is much more difficult and much less predictable.
