After going through a few interviews, I am having a difficult time interpreting how the interview went and if I have a chance at these programs. I have spoken with friends applying to other specialties and they have gotten more straightforward signs from the interviewers.

Will most programs give you a sense that they are/aren't interested in you during or after the interview? Any suggestions on determining how the interview went?

In a word, don't.
You will likely interview with between 8 to 20 people during that day and even if everyone loves you the responses will be different.
In addition, there is another day of people you'll never see with whom you are being compared.
After we have met you we will meet a few days later to try to get our rank list in order while we still remember everyone.
Some will get ranked high because they are stars, others will climb the listing because they seemed to be nice people who were enthusiastic and not pushy and seemed capable.
Others will get on the list becasue they made a connection with one person who went out on a limb and pushed for them.
You will drive yourselves nuts trying to guess how you did.
Take it easy, enjoy the day and make sure you have your questions answered.
Do not try to figure out how you did.
When you make your rank list, put down the programs in order of how badly you want to go there. Getting your number one choice only matters if you want to be there for five or six years. Do not rank a program you think you will be miserable in unless you have no other choice.
