Interested candidates should submit the following information:
- Current CV
- Copy of medical school transcript
- Personal statement including reason for leaving current program and research publications
- ACGME operative case log to date
- Copy of PGY-1 block schedule, including rotation descriptions and dates
- Copy of OITE scores
- Milestone evaluation summaries
- Recommendation letter from current program director verifying current standing in the program
- Two additional letters of recommendation from faculty in current program.
PGY 2 applicants (Start date July 2023)
PGY 3 applicants (Start date July 2023)
