What I found helpful during the intern year was an "On Call: Principles and Protocols" book published by Saunders. I believe they have a whole series now including a general book as well as subspecialties. It was useful to use when the nurses called in the middle of the night with something like low sodium values, shortness of breath, chest pain, etc... It gave a nice little protocol to follow. It still sits on my shelf now and occasionally I pull it out before I call the internist for a consult.
If you are looking for something more ortho to read, then I would suggest spending the time to read the Orthopedic Basic Science Book
www3.aaos.org/product/prt_item.cfm?code=02452
It will be a little slow reading, but you will rock the basic science section on your OITE for the next five years. In addition, I would suggest a basic ortho text such as Orthopaedic Surgery: The Essentials to give you a broad background in orthopedics. It will layout a base of knowledge for you to build on with more detailed books as your progress in your training.
