For programs only asking for 3 letters (but not necessarily asking for a maximum of 3 letters) is it a good idea to submit 2 letters from attendings you have had clinical experiences with plus a chair letter? Or is it better to submit all four letters and let programs ignore one of them if they want to?

My plan: If their program website states "three letters", send three letters.

If a program's instruction say "minimum of three" then four is ok.

Picking between a department letter and personal LOR is difficult. If a program specifically asked for it they got my department letter. Otherwise, I went with a personal letter which I feel will speak more about character and personality.

My two cents.
