Depends how novel/unique it is. If you want to go spine specific, Spine, The Spine Journal, Journal of Neurosurgery: Spine are probably the most competitive. Clinical Spine Surgery is good too. After that can look at general ortho journals like World Journal of Orthopedics, American Journal of Orthopedics, International Journal of Spine Surgery (not on pubmed)
