Letters of Recommendation

Hello everyone,
I am a 4th year student applying into orthopaedics this upcoming cycle. I have 3 strong letters of recommendations from orthopaedic authors. I know most places accept a 4th letter, and this is the one I have a question about.
My most recent rotation I was at didn't lend itself to earning a strong letter. I worked with a different physician almost everyday. The doctor I think I hit it off most with is who I asked for a 4th letter, but I'm not expecting some fantastic letter, more lukewarm, simply because I only worked with him for a total of 8 days.
My options that I see are:
1) Submit only the 3 strong orthopaedic letters.
2) Submit all 4 orthopaedic letters (including the possibly average one).
3) Submit the 3 strong orthopaedic letters and a 4th strong letter from a general surgeon.

What are everyone's thoughts on how best to proceed! Any advice helps. Thanks.
