I am a medical student considering orthopedic surgery, and if so, likely sports medicine as a fellowship. I was wondering what the lifestyle is like (call, hours, family life, etc), and how happy are those who have chosen this as a career.

Most every orthopedic surgeon does sports medicine in some capacity. The lifestyle will be what you make of it. You could work a 40hr week or an 80 hr week. If you want to become a team physician at a collegiate or professional level, then I guarantee that you'll be a lot busier and work later as most of those duties are carried outside of your regular practice time.
