Please, please, please pass this on to everyone applying in orthopaedic surgery. Through the CORD (Council of Orthopaedic Residency Directors) we created a single universal supplemental evaluation form for the 5 programs in the country that require them (Brown, University of Washington, Vanderbilt, Washington University, St. Louis, and Yale). We did this to make your lives easier and so that you would not have to ask your attendings to fill out 5 different forms. Somehow, this information has not gotten out to everyone and lots of students are sending multiple forms to attendings - please make sure that you share this message with all of your peers around the country who are applying this year. Thanks wnl
MCW and UCSF-Fresno (1 letter writer only) are also requiring the form this year.

What is the best way to get the Universal Supplemental Forms to programs? I've seen on some program websites that they want them through ERAS with your Letters of Recommendation and other schools say you can email them directly. If your Letter Writers put them with the letters, is there a way to keep it separate so that schools that are not requiring the forms don't have to see them? I know you can only send 4 LoR documents to each program. I'd hate to have to ask my letter writers to upload a version with the form and one without.

Of the places I applied to last year, these programs required the supplemental evaluation letter: Cincinnati Yale WashU St. Louis Mount Sinai Brown Vanderbilt UWashington Here are other programs with extra requirements: Case Western: Copy of undergrad transcript if you're invited for interview Iowa: send them an email with a paragraph about "why iowa" UVA: Supplemental application (not the letter, it's an application you download from their website) UCSF: 1 of your 3 LOR must be from an attending who isn't a surgeon, also require passing scores on Step 2CS and 2CK by February 1st UCLA: send MCAT scores via email Rutgers-Newark: send them MCAT scores via email NYU-HJD: include a paragraph about "why NYU" in your personal statement on ERAS, so you need to make a separate PS for them Harvard: passing scores in Step 2CK and Step 2CS by January
