I've been coming up with the same results from websites, FREIDA, etc. Regarding the supplemental LOR forms - at least five of the programs I'm applying to require them. Does anybody know how strict they are about having a form returned by EVERY letter writer? I'm just trying to figure out the best way to ask attendings for letters, and then say, "Hey, would you mind filling all these out too, in addiiton to the eval from my home institution?" Especially the letters that were written for me a few months ago, before I knew where I was applying. How are you guys doing this? Am I making this more difficult than it's supposed to be???

i think that medical student performance eval is what was formerly known as the "dean's letter" just fyi...

Not to be a tool, but I think you just have to ask. I've had to do the same thing, it's a bit weird to do. The way I look at it, I'm not the world's strongest applicant on paper (avg Step 1), so the least I should do is get all the paperwork in. Another way to look at it is that if 80% of the pool got everything in and you didn't...

Yeah, I knew the answer, I just didn't want to think about getting the forms to my AI letter writers. Oh well, I'd rather suck it up now than on un-match day... Thanks for the advice

Hey. My list (taken from department websites) pretty much checks out with what's above. I found a few discrepancies: University of Pennsylvania: No college transcript University of Iowa: No MCAT/SAT scores Also, some other programs with supplemental requirements: UC Irvine: CPR, ACLS, BLS certificate prior to interview date Case Western: College transcript If earlier posters have different sources of information for supplemental requirements, please feel free to correct me on the Iowa and Penn info. Hope this helps. Not looking forward to asking for 4 to 5 supplemental forms to be filled out. Especially from letter writers at away institutions.

for the programs that say they want a CV, do they mean just the ERAS version of our CV, or do you think they want a hard copy of our regular CV sent to them? a few programs I came across that list CV in their requirements...... Vanderbilt, Mayo, UPenn, Yale, Cincinnati Thanks!

Though UPenn site does not say anything about a college transcript, the letter that they emailed me when I requested info says they do require a college transcript. So, looks like they do want the undergrad transcript.

Doing a search on old posts is not accurate since program requirements may have changed for this year. 1. in Eras there is no choice of 6yr vs 5yr for the UMass program. Their application requirement website (http://www.umassmed.edu/orthopedics/residency/apply.cfm) does not state the requirement to indicate 6yr vs 5yr in the PS. Can someone provide some more advice? 2. In addition, UNC wants only TWO LORs (http://www.med.unc.edu/ortho/RESPROG/letter.html) 3. Iowa is not requesting MCAT and SAT scores but does want college transcripts (http://www.uihealthcare.com/depts/med/o ... l#2003info) 4. agree w/ ortho-pod on the UPenn college transcript requirement. I checked up their website a few months ago and the college transcript was a requrement. But that requirement has been recently deleted since I can no longer find it. 5. The cleveland clinic does have an application form but this should not be required if you're submitting an ERAS application. I believe this is more for a Fellowship application. in response to jorthox I was just going to submit the ERAS application thinking that the ERAS CV is good enough for the "CV" requirement. I called a few programs (U Penn, CASE, Vandy, U Michigan) and the concensus is that ERAS CV is good enough. You can send a separate CV if it contains info that is not in the ERAS CV. Good luck to us all!

I talked to UPenn's coordinator today via phone. She told me that they no longer require an undergranduate transcript starting this year (which is why it is no longer on the website).

Forgot to include above, UPenn does not require an additional CV. The ERAS CV is what they want. This is from a phone conversation with the coordinator there.
