I applied to the match a couple years ago, right when it was instituted so I feel like I have pretty good first hand experience.
It isn't anything like residency application first off. The community is tiny relatively and you don't need to apply to every program.
Some of the fellowships I know about:
Union Memorial in Baltimore: A really good program with good variety covering the entire spectrum of foot and ankle care. Schon might be the most well known but word is that all the people you work with are great.
Idaho foot and ankle fellowship: Dr. Coughlin is about as well known and respected as anyone. Boise is a cool place to live for a year. Can't go wrong.
Charlotte: With Dr. Anderson as president of the AOFAS, you will be well connected. A great all around program.
It sort of depends on what you want out of your fellowship - a mentorship with one person or a potentially broader experience but with the caviet that it will be more like a 6th year of residency.
